1. Promoter & Sponsors The Promoter is CW Media Pty Ltd (ACN 606 548 298), 44-54 Raglan Street Preston, Vic 3072, on behalf of The House of Wellness.
2. Promotion States: All Australian States
3. Duration of Promotion: The Promotion commences at 12:00pm AWST on 22/09/2018 and will close at 11.59am AEST on 30/09/2018 (“Promotion Period”).
4. Advertising of Promotion: The Promotion may be advertised through the Promoter, its advertising agencies, marketing affiliates, the Sponsors and selected charities.
5. Eligibility: Entry is free and is open to Australian residents (“Eligible Entrant”), excluding any employee of the Promoter, the Sponsors, or related companies, and any child, parent, spouse or sibling of such employee. Participants under the age of 18 must first seek permission from their parent or legal guardian.
To enter, each Eligible Entrant must complete the following steps, as detailed below:
a) To enter, the user must fill out the entry form and include a response to: Tell us in 25 words or less why you want to transform your life.
b) It is the responsibility of each eligible entrant to ensure that their details, including their phone number, email address and Postal address are correct and up to date when sharing with the promoter.
Ten (10) winners will receive one (1) prize pack each valued at RRP $29.99. Each prize pack is as follows:

The total prize value of the promotion is $299.90
The prizes outlined above must be taken as stated and no compensation will be payable if a winner is unable to use the prize as stated. The prizes cannot be redeemed for cash and are not transferrable. If the winners are unable to accept the prize, there will be a re-draw. Prizes include postage and handling (if required). The Promoter is not liable for any additional costs that may be incurred by the winner as a result of this prize including but not limited to interstate travel, accommodation, entertainment, meals, transfers, etc.
a) Judging will take place at AM Solutions 44-54 Raglan Street Preston, Vic 3072 at 12pm on 01/10/2018.
b) The winning entries will be judged on creativity and originality and is subject to the discretion of the judging panel.
c) The winner/s will be notified on the competition post within 24 hours of their entry being chosen.
d) If the prize is not claimed by 5:30pm by 07/11/18 the winner’s entry will be deemed invalid and the Promoter reserves the right to conduct further judging at 9:00am on 12/11/18 at the same location as the original judging as are necessary to distribute the prize.
e) If any redrawn prize is not claimed by 12/12/18 the prize will be forfeited.
f) The judges’ decision in relation to any aspect of the competition is final and binding on each person who enters. This is a game of skill.
g) Chance plays no part in determining the winner. All entries will be judged individually on merit. No correspondence will be entered into.
9. Notification: The prize winners will be notified via email by a Chemist Warehouse staff member within three (3) days of the Promotion Period ending.
10. General conditions
a) The Promoter may require the winner to provide proof of identity, age, purchase receipt number, and proof of residency prior to awarding the prize.
b) The winning entrants authorise the Promoter and Sponsor to use their names and photographs for publicity purposes and agree to reasonably co-operate with the Promoter in publicising the Promotion.
c) If at any time the Promoter considers in good faith that an entry breaches these Conditions of Entry, the Promoter reserves the right to refuse an entry into the draw at their discretion. The Promoter’s decision is final and no correspondence will be entered into regarding the decision.
d) If a winning entry does not comply with these Conditions of Entry, the entry will be discarded and a new winner of the unclaimed prize will be determined by drawing a further valid entry.
e) Each Eligible Entrant agrees that by entering into this competition, the Promoter and Sponsor will be permitted to market the Promoter’s and Sponsor’s products and services to the Eligible Entrant using the Eligible Entrant’s entry details.
f) If for any reason any aspect of this Promotion is not capable of running as planned, including by reason of infection by computer virus, mobile network failure, bugs, tampering, unauthorised intervention, fraud, technical failures or any cause beyond the control of the Promoter which affects the administration, security, fairness, integrity or proper conduct of this Promotion, the Promoter may in its sole discretion cancel, terminate, modify or suspend the Promotion, or invalidate any affected entries, subject to the approval of the relevant authorities in each state and territory, where required.
g) Entrants acknowledge that this promotion is in no way sponsored, endorsed or administered by Facebook or Instagram, and that any queries, comments or complaints about this promotion must be directed to the Promoter and not to Facebook or Instagram. In participating in this promotion, entrants are providing information to the Promoter and not to Facebook or Instagram. c) The Promoter or Sponsor accept no responsibility for any entries not received during the Promotion Period. No responsibility will be taken for lost, late or misdirected entries. The Promoter does not warrant that the entry mechanisms will be available, or the Promoter will be open, at all times during the Promotion Period.
I. Promoter, the Promoter’s related entities and all agencies associated with the Promotion; and
II. The employees, agents, directors and contractors, of all entities referred to in paragraph below;
(I) shall not be liable for any loss or claim, action, demand, liability, damage, cost, expense or personal injury whatsoever (including but not limited to any direct, indirect or consequential loss), incurred, suffered or sustained by any person or entity (without limitation) in connection with, or arising from, the promotion or the acceptance or use of the prize, except that which cannot be excluded by law (in which case that liability is limited to the maximum extent allowable by law).
12. Collection Notice
a) The House of Wellness collects your personal information (including name, phone number, email address and membership details) when you enter this promotion for the purposes of administering the promotion. If we do not collect your personal information you will not be able to enter this promotion or claim your prize. We may disclose this information to third parties such as local lottery authorities and service providers assisting us with administering this promotion, for the purpose of conducting this promotion, including prize notification and fulfillment.
